Below are all release forms students need to complete…
- Central Health History and Medical Release – Authorization for emergency treatment of a participant in the case that a parent or guardian cannot be reached. All participants at overnight events must complete this form once each school year.
- Central Overnight Event Permission Release – This form must be completed again each time a student attends an overnight event sponsored by Central.
